Appointments

 

Your child’s scheduled appointment time has been reserved specifically for them. We request 24-hours notice if you need to cancel their appointment. A cancellation fee of $50 will be charged if less than 24 hours notice is given.

 

Insurance

 

Our Preferred Providers are

 Delta Dental, Regence, Premera, Lifewise, and Lifemap

 

We bill to most major insurance companies and our staff continually strives to stay on top of any changes that occur, but it is impossible to know the details of every policy.

 

We encourage you to contact your insurance company, to become familiar with your benefits. As a courtesy, we verify active coverage before your child’s scheduled appointment, however, this does not include an explanation of benefits. If your plan reveals that it is not-active for the day of service, we will request payment for the services provided at the end of your child’s appointment. We can provide you with an explanation of services to submit to your insurance company for reimbursement.

 

Any services not covered by insurance is the parent’s financial responsibility, along with following up on insurance disputes or questions with coverage.

 

Let our front office staff know if you would like assistance in estimating your benefits coverage, or any out-of-pocket expenses prior to your child’s appointment. We can submit a pre-authorization to your insurance company, but please keep in mind their response can take up to 2-8 weeks.

Financial Information

 

 We accept payments by cash, checks, and all major credit cards. Please ask a member of our staff, and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.
